Alvarado Score (MANTRELS)

Evaluates the probability of acute appendicitis based on 8 clinical criteria.

Symptoms
Migration of pain (+1)
Pain migrates to the Right Lower Quadrant (RLQ) / Right Iliac Fossa.
Anorexia (+1)
Loss of appetite or presence of ketones in urine.
Nausea or Vomiting (+1)
Patient experiences nausea or has vomited.
Signs
Tenderness in RLQ (+2)
Tenderness in the Right Lower Quadrant / Right Iliac Fossa.
Rebound pain (+1)
Pain upon sudden release of deep palpation in the RLQ (Blumberg's sign).
Elevated Temperature (+1)
Fever greater than or equal to 37.3°C (99.1°F).
Laboratory Values
Leukocytosis (+2)
White blood cell count > 10,000 cells/μL.
Shift to the Left (+1)
Neutrophilia > 75%.
